Class of 2030 (incoming Freshman) Information Form

All incoming freshmen are asked to complete the Incoming Freshman Information Form (if you have not already done so). This form helps us collect important student information as we prepare for the upcoming school year.

Next Steps for Accepted Families

• New families: Check your email for your Plus Portals activation link.
• Current SJB families: Access your Class of 2030 student under Course Requests in your existing account.

• Course request instructions
• Upload your child’s most recent MP3 / Trimester 2 report card to your Admissions Account: https://forms.rediker.com/stjohntbdhs
• If already submitted, email the updated version to [email protected]

• A Course Verification form will be sent in early June for your review.

• No action needed. Final report cards will be requested by our Guidance Department.
