New York State Education Law requires the following:

• All 9th Grade Students and any New Students must have immunizations and a valid physical to enter St. John the Baptist
• All 11th Grade Students must have a valid physical.
• All 12^th Grade Students must have a booster of the Meningococcal conjugate vaccine.
• All 9th graders, 11th graders, and new students will have a hearing and vision screening performed by the health office.
• All Student Athletes need a valid physical to tryout for and participate in any sport.
• In addition, occasionally it is necessary for a student to receive medication during the school day. The school nurse will cooperate with parents and physicians in these situations; but we are obligated to follow precise procedures. In order to comply with these guidelines from the New York State Practice Act and provisions of State Education Law, the following requirements for administering medications in school include:
• A written request from the family physician in which she/he indicates the drug, frequency, dosage and method of administering.
• A written request from the parent to administer the medication as specified by the family physician.
• The medication must be delivered directly to the school nurse.
• Medication forms can be obtained online.
